Stupid question: does anyone know how The Speaker died exactly? He falls out of those shackles and is just... dead. Or did I miss something? Was it a culmination of his injuries or what?

If Tess was captured, the campaign would have likely been super short. Instead of the drawn out probing of the Speaker for how to get recognized by the Traveler, Ghaul would have known exactly what it would take to get what he wanted: throw money at the screen.
One plot element would have remained the same: Either way, Ghaul would have strangled his adopted dad. In the "kill Tess" version however, he would have been strangling him to get unlimited use of daddy's credit card for those lootboxes.
The bottom line is: If Eververse was the endgame for Ghaul, we would have all been screwed, as he would have nuked our sun much sooner... unless, of course, he just had the worst case of perpetual RNG-madness with his lootbox drops that ever hit the planet. Something like... Ghaul: "I've spend 3 billion dollars on lootboxes and all I got was this lousy shader."
Should have sent the Eververse to Ghaul.
Either Ghaul would have obliterated Tess, in a very messy and final manner...
Or she would have given him and the consul a much-needed makeover.
