Who would you save, and why?
OK, there is only time to save one. You have an elderly person, dying of cancer. They WILL die in 5 days, and it will be painful. No medicine can ease their pain. or: There is an unrepentant serial child molester. He will never be reformed. Finally, there is a politician.
Who gets saved?

Save the politician. They're scumbags, but at least they're not child molesters and the elderly one will die painlessly. Hopefully. My only regret is that the molester will also get a painless death.

Probably the politician. Yeah, politicians are scum of the earth, blahblahblah. The old person has no utility and brings nothing to the table. The child molester has proven he doesn't deserve to be here. The politician has some influence in the city/state/country to make it a better place, assuming he's not a corrupt asshole.
