Lack of focus is a huge reason some teams are just horrible. I've had people trying to take videos of stupid shit, eating during boss fights, busy posting shit on facebook or instagram in the middle of a boss fight, talking on the phone, random afks without a word.....too many things to count really.

The inability to communicate due to constant yelling and cussing by 3 raiders that were apparently friends in real life. It took us about 4 hours to get from the start to golgoroth because of their idiocy.

The little kids with ADD that can't handle it being silent for more than 4 seconds The little kids that decide to be a human echo to short side conversations that don't relate to them in any way. The guy that joins and says he knows how to do oryx and upon start and wipe at first orbs it becomes clear he hasn't got a -blam!-ing clue, then proceeds to beg you to carry him through it.

I personally think it is people who have been carried through certain spots of the raid or have gotten lucky when they finished it the first time make team cohesiveness go down. I also think that people who join and say they've done it before but either haven't, or aren't confident in themselves and mess up the flow of the team because they expect someone else to pick up the slack.

In my experience, every raid team needs a commander to call the moves and cadence. An average team will succeed with a good commander. When no one is calling the shots, it ALWAYS fails.
