If the knife is thrown somewhere that you cannot teleport to, the teleport does not happen.
You would need to attach a clip to be able to tell if you’re encountering a bug or if it’s working as intended.

This happens to me too during onslaut: salvation (widow's court) boss. It only throw knifes 3 times without the blink attack. This super does really bad dmg because of this.
