I’m not too big on Fallout 4, but to my knowledge, yes, some are just robotic copies of existing people, while some are entirely new ones. You also have different “levels” of synths with varying levels of sentience.

They are robots underneath all the flesh and blood, but they also have a real biological component to them because DNA from the leader of the Institute is used to make them... Or something like that. Unless you're talking about gen 1-2 synths. Those are just total, mindless robots. Gen 3 synths are the human ones
