how do you get a flaming helmet.

Get a time machine (preferably a TARDIS or a DeLorian) and go back to around June/July 2011. Download the Bungie app for iOS, and activate the Bungie Nameplate. Then go on bungie.net and activate it. But you can't now. The only way to have them now is to have had the Bungie nameplate active when Bungie gave Halo Reach server control to 343, at which point you could no longer get nameplates.
