I completed the quest, and when I went to another character to do another looted run, the focus chest didn't give me the scout rifle. I entered worm give guardian in the first area with all the hidden, then activated the brain symbol in the worm room. when I went to loot the chest it didn't give me anything and had an entirely different symbol hovering over it.
