I have played several Crucible matches (momentum control, but it has happened in other gamemodes as well) where my progress is not saved at all and its infuriating especially since those games i get multiple rapid defeats using a grenade launcher. I am happy that i get them, then crushed when my progress on mountaintop is not saved at all. Please for the love of god Bungie, do something about it. This happens way to often, only today i have had my progress wiped a whopping three times. I've read about it and it seems this has been happening for years. I can tell when it happens because i only get 1-2 solstice fragments on match completion instead of the usual 14-16 and the valor overview to the left doesn't popup either. I can also see that the game has registered the match on both destinytracker and on bungie.net.
