One time on Halo 5, at the beginning of the match, the opposing team spammed grenades at the path I was running; there must have been six grenades under and around me. I exploded and my Xbox crashed at the same time. It was pretty funny.

If you have sunbreakers and activate radiance (with solar grenades obviously) and you throw as many as possible your game can also crash.
