There's a ghost near where you start the game. I'm not talking about the one that came in the house of wolves. I'm talking about the one in the room where you kill many assassination targets for patrol missions. It's called Old Russia 2 to be specific. I explicitly remember picking it up, but there may have been connection issues or something that caused it to not be present in my grimoire, and I'm pretty neurotic about my grimoire score. I would have provided screenshots but this forum only allows links for some odd reason. Am I the only one with this ghost issue? Is there a way for Bungie to simply fix it?
