For flawless raider can not a single person die at all during the raid or can the whole team not wipe during any point in the raid?
Or in other words can we still get the achievement of people die but the whole team doesn't wipe?

No one on the team can die. A disconnect can sometimes work. You can also kick members and still get the trophy.
