The moon one is off a ledge in circle of bones, the venus one is in the ember caves on top of some rocks.

Aren't those Moon Ghost 4 - Ghost Fragment: Saturn and Venus Ghost 12: Thorn? It looks like it. It seems to me that the count is glitched, perhaps if you grabbed one of the glitch ghost prior to the dlc it somehow counts those as well. If I'm not mistaken there were 2 glitched ghosts at that time, I remember picking one of them, hence why it says I have 69 rather than 68.
