Might be the settings in your DSL / Cable modem.
Try setting it to BRIDGE MODE if you're plugging it into a router.
Also, LinkSys routers are terrible for this ... I actually have to plug my XB360 directly into my modem to get an open NAT. With all the settings in the router, I was never able to get anything better than a moderate NAT and still had issues frequently.
