I got it last week and the achievement unlocked from one fully clean run through. I know that even if one person dies in a non restricted spawn area (jumping down from the damn templar........) it will not pop. I've heard if someone lags out you can still get it but they can't join back in so it'd be best to just restart if you actually like that person
