Joker is by far the worst of the worst but I would say Ra's al Ghul and Bane are next in line.

Ra's al Ghul is basically an immortal and is in control of an army. Bane has intelligence that can rival Batman's and his Venom makes him much stronger. Both also know Batman's secret identity.
