Now, I know in the Halo Series people formed clans/guilds outside of the game and played together that way. Did any game of the series have tags or indications that that person was part of a group (other than having "[CLAN]Playername" as your game name)?

I do believe they'd also all have the same emblem.

Not that I know of, beyond Bnet groups. I'm not an avid clan member, only time I've been in them is when I'm with people I know in real life. What I meant by clan in this post wasn't an external fan group, but rather an in-game alliance of players.
