We understand you didn't receive an exotic cipher after resetting Xur. Please be advised that the maximum exotic ciphers you can hold is five. If you already had five in your inventory, then that would be why you didn't receive another cipher in this instance.

I had no Exotic Ciphers left in my inventory, only 3 Forsaken Ciphers. As a returning player, I used all I had buying other items over the last 2 weeks.

GrundlBeans commented in this thread that they also disappear if your item inventory is full. This seems like something that should be blocked out similar to when you try to claim an Ascendant Shard, but if you have max stacks and cannot. Considering the multitudes of Eternity junk tickets and destination items that may show up without you realizing, as well as deprecated destination materials for those of us returning, this could be an easy solution. In my case, I freed up space a couple of days before, but before going to Xur and ending my gaming session, I unknowingly had new Pale Heart items fill up my inventory. Thank you again for your time.
