We understand Dyadic Ascension appears bugged. We're sorry to hear that's the case, as we can imagine the concern caused by this. For our investigation, could you kindly record a video which demonstrates you visiting the osseous fragment locations, but the 18th one doesn't appear?
Once you have footage, you can upload it to a site like Youtube, and we'll review.

Hello again, Thank you for this footage. Having reviewed the provided video, it appears as though you may in fact be missing a fragment from the Mausoleum. We encourage you to continue exploring the Mausoleum and other locations related to this quest. We've no doubt that by doing so, you will eventually locate the fragment you're missing.

I can assure you it is bugged. I spent 3 hours looking for the last piece. I literally walked around the entire area.

Guys it literally is bugged. I'm stuck on 16/18 fragments and I've retraced my steps 6 times in a row now and I for sure have them all but my progress doesn't show that.

I retraced my steps again and the last one magically appeared today in a spot that I have checked at least 12 times so I guess keep looking dude.
