In the same boat here... The Mindcraft triumph somehow doesn't count all the weapons, despite me having 2 of some. I'm also experiencing the same issue with the Master Vox Obscura triumph. I have the catalyst but somehow it doesn't get checked. :(

Did you let all of the dialogue get spoken? that may be an issue.

Hmm, might be it, though I’m pretty sure I did. The second run I finished was on Master and the triumph for the unique dialogue [i]did[/i] increase.
