IKO-223 is just the hidden designation for Ikora. It’s the format Hidden agents use to identify themselves in messages.

IKO-006 is used in the Mnemosyne lore book and it is her designation in Ikora 1 of this lore book. I assume that having 2 designations is unlikely

Yes…but Mnemosyne’s decryption key is set to IKO-006, and I believe the agents call # isn’t 006

Indeed, it’s a curious detail. Something like Stolen Intelligence is also interesting, as each report there shares a decryption key ending with Aunor’s designation. Makes me wonder if it’s a typo given that it’s largely considered that the last report is one of the two within the book that isn’t a forgery. Or perhaps it is that hybrid decryption keys exist.
