I believe the you either have to have the whole team go deathless or do the entire run from beginning to end deathless. I would try those two things if you have not already. Some believe it is only the boss room, but I would try and do the whole thing.

Both times deathless from start to the end. Ppl on reddit say they got it even when teammates died. It would be impossible to have flawless teammates included.

Hmm, I could not find anything related to this specific topic. If it were me, I would try to make sure you do both bosses flawless again, maybe even trying tier 2 reckoning. If that doesn't work, I would post a bug in the issues part of the forum.
