Hi there,
Thanks for your report. Did this occur multiple times in the same run or across multiple dungeon attempts?

it occurred across multiple dungeon attempts somehow reset the check point and it didnt fix itself

You never touch the plate in the center of the main stage where the acolytes are praying. If you go too long without touching that plate, it wipes the group. Notice that your cause of death is "Annihilator" which is typical for those plates with the spiky three piece totems over them.

That was my first thought too but i did try by going over the plate and still got wiped randomly
