Hi there,
Thanks for your report. Have you been able to load back into Last Wish from the checkpoint and complete the Queenswalk successfully since this occurred? Or is this continuing to block your team from completing the raid?

Currently ran into this issue today. Happened to multiple members of the fire team where we could take Riven's Heart into itself.

Issue is still present where Queenswalk would still do this, date is 01/10/2021 today will post video ASAP

Heck, I didn't see the notification. Yeah, we were able to complete it later in the week. It happened a couple of times, but we got lucky enough for it to not happen once and beat it.
