It's the matchmaking and the matchmaking alone.
SBMM will keep you at a 50% W/L and a 1.0K/D, because if you play well, you match better players, and if you keep playing well, you'll continue to match better players until you match people you cannot beat, and then your stats plateau.

This isn't a coincidence, this [i]is[/i] the reason. This is what SBMM does.
There is no "gameplay manipulation." Most of the things you listed can be explained by latency or lag, and the enemy team catching up is either you just not paying attention to the score or you playing a mode that is easy to come back in (i.e Control or IB.)
SBMM also means more lag for everyone, but hey, this is what people wanted, so.

Also one thing people fail to understand. When you do good in a match, all too often I see the same players who pulled 37-2 last match go 10-8 because they get ridiculously overconfident, play too aggressive, underestimate everyone, or just play against someone who's running an effective counter to their loadout. But the main thing, people are too confident after winning.
