All you need to do is pull up your game history on Bungie.net and find a qualifying Crucible match and copy a link to the PGCR and post it in the Help forum.

Okay so I saw that I played crucible the week Prometheus lens could melt guardians, but I didn’t play that specific weekend. That means I’m not eligible doesn’t it?

Correct. You had to play between 10am Dec 8th to 10am Dec 12th 2017 Pacific Standard Time to qualify for the emblem.
