According to the Lore the Drifter maybe suffering schizophrenic episodes or perhaps PTSD. It seems that he has seem something traumatizing...
*Edit* He is also hungry, he always seems hungry, that is a symptom to anxiety...

Pretty sure he was talking to his ghost. In the new lore, they explain in great detail that his ghost is [i]really[/i] good at hiding. “Being hungry” also seems like an inside joke between the two, since Shin died of starvation multiple times as a young risen.

That sounds right, from what it says in the new Lore it says that the Drifter himself can summon the Taken (or something similar) on whim.... This close connection maybe a stresser and could be harming his mental stability...
