Ok so you are on a notebook with a mobile nvidia chipset. Make sure that your notebook is ACTUALLY using the graphics card and not just your integrated graphics (Intel onboard). This is a common problem especially after updating your graphics drivers. In game, what does it show as your available video memory, that will clue you in if it shows your available graphics memory as like 768 MB.

Same thing though. Make sure your monitor is connected the proper port and double check to make sure it is using your dedicated graphics. Your CPU-Z is indicating otherwise.

My gaming monitor is plugged into my graphics and I have my alt monitor in my MOBO intel graphics.

That is more than likely what is causing it honestly. Unplug that and go to one monitor and see if it resolves it.
