Out of about 30 or so attempts we were able to get to damage phase maybe 3 times

Yes. We made sure of it. Randomly it would let 1 person destroy the ship but not the second. Other times no one could destroy a ship. We were doing it fairly quickly considering we had enough time to try it 3 or so times to make it work.

Hmmm. Maybe next time it happens, have that person (or those people) record a video of it occurring. That may provide more insight into it. I personally haven’t noticed this, except when the person going to orbit fails to get greed.
