Hmmm, "Heavy" can be quite subjective.
I'm going to take the music, not lyrics, path. We're talking HEAVY here, not fast or aggressive, just pure heavy.
Here's a few
1. Sunn 0))) - Hunting & Gathering from Monoliths and Dimensions
https://youtu.be/-V4QMrc5NAI
2. Electric Wizard - Funeralopolis from Dopethrone
https://youtu.be/F-x6qqNy0mo
3. Sleep - Holy Mountain from Holy Mountain

And I haven't even touched the more obscure bands, it just depends how far down the rabbit hole you wish to go.
1. Sunn 0))) - Hunting & Gathering from Monoliths and Dimensions https://youtu.be/-V4QMrc5NAI
I've heard people mention this band before here and there, and this is my first time actually listening to them. I'm gonna be honest, I'm not really a fan of Drone Metal or whatever genre these guys fall under. It's too slow and repetitive, and the fact that there's no drums at all (at least from what I could hear) is kind of a deal breaker for me.
2. Electric Wizard - Funeralopolis from Dopethrone https://youtu.be/F-x6qqNy0mo
I liked this one more than the Sunn 0))) track, especially when it started to pick up during the second half. I still don't think it's something I'd go out of my way to listen to though.
3. Sleep - Holy Mountain from Holy Mountain https://youtu.be/Y4cxaVEqZsE
Again, this has the same problem the other two have. Personally I just think it's too slow and boring. The riffs have no impact and just kind of, well, drone on. Personally, if I want slow and heavy, then it has to be Black Tongue.

Interesting. Well, like I said, "Heavy" is subjective. To me, Black Tongue is not heavy, they're kinda middle of the road metal (I'm not knocking Black Tongue though). For me, when someone says "Whats the Heaviest?", I think of a deep, crushing wall of sound. Speakers tearing at the seams because the key is dropped so low, walls crumbling and bones shattering. Since you're looking for a little more melody (yes, I'll admit you do start to lose melody the heavier you go), try this one on for size, https://youtu.be/aUmMh9NAWX4

I guess this comes back to the perspective thing we were talking about, cause when someone asks me that question, I think of slow, heavy, crushing breakdowns where you can feel the riffs shake your body and the drums sucker punch you. I guess what I was trying to say about Sunn 0))) and similar artists is that there isn't that moment of "impact" for me. You need something to contrast the heavy so when it hits it hits hard. I feel the same way about genres like Grindcore. If your whole song is just gravity blasts or a drone then it's going to get boring. I think melody can really enhance a breakdown, and Black Tongue does it really well. They include these reverb-y lead guitar melodies that just instil a sense of horror and despair. If you like downtuned stuff, I found this song a little while ago. The video is a little cringey though. https://www.youtube.com/watch?v=WbQbwmZU2To
