Dear Guardian,
Thank you for reporting this bug here in the forums. I would recommend adding in any relative data like videos or other specific information as the more information and perspectives you can provide, the better chance there is to recreate and isolate the conditions that caused the problem.
Sadly, there is no direct way to fix this bug, but to prevent it in the future I recommend planning out your damage phases so that it does not come down to a Damage Over Time effect as the raid will do the wipe mechanic and register that as complete and then you kill them so it attempts to register that as well. I believe that this is where the error is occurring so avoid it at all costs.

Actually I know how it may have happened. My fireteam was on the shield part where he was a hair.close to death. My team dialed to destroy the shield and last second my teammate threw a grenade that killed him but it didn't respawn us. Mabey that's how it happened.

And where do you want me to send the screenshots

Sadly, Bungie does not generate items in game for any reason: https://www.bungie.net/en/Help/Article/12211 Also, if the achievement did not pop, then the game does not register that you defeated Callus and you will need to do it again.

I don't think they'd help but it would show me and 5 other guardians dead and it saying waiting for revive
