After about 60 hours I beat the game and, right after I beat the game I fell through the floor in the tempest and was stuck in a tiny black box. I couldn't fast travel out so I had to reload the last autosave which was right when I was falling through the floor. It was impossible to get out, so I had to reload the save before that which was the start of the last mission.

Something kind of similar happened to me on a loyalty mission. It was Jaal's and you had to go to the Roekaar Camp and meet Akksul. I reloaded that save, but all I could do was just watch my poor female Ryder fall through the world over and over again.
