Normally, when that happens, it's because of lag of some type, so is possible that either someone else was loading into the area and the synching was hiccupping or someone already in the forge was lagging to whoever the host was. When that happens, it's usually best that everyone start dying, one by one. When the person causing the hiccup dies, it should continue on.

In the video I checked connections. Both me and the random had green bars. On top of that, we killed enemies just fine, and they spawned fine. Everything was like a normal day. My first reaction was lag. As is everyone else's. But I do assure you everything was 100%. I just think it bugged out for once.
