Once there are 3 or more members in your group, you may select the button at the top of the group page that says "Set as Xbox Clan" or "Set as PS4 Clan". You may only select 1 group as your clan. Some groups require clan approval, where a request will be sent to the admins, and they can accept you into it.

There are 18 members and I have been accepted but there is no set as clan button.

Then that group must have the clan feature set to "Invite only." You'll need to be invited by an admin to join.
