Onion Bro dies no matter what after Yhorm.
His quest line is complete and for your service to said Onion Bro you get:
His armor
Storm ruler
English

you are supposed to get a Slab as well

The slab is after getting him out of the cell in irithyll.

I never got one cause I don't do the quest line but according to dark souls wiki you should get one for doing his quest line

The only time you don't see him die os if you kill Aldrich before Yhorm
