You can tell MGSV was a labor of love for Kojima - the amount of content in that game is absolutely amazing.
If you do not have the backstory (or any of the story) then you should get an overview somewhere to really grasp what is happening. The overall story for MG is dense, and if you are coming in fresh it will be easy to get lost. Plus the ending will not hit youthe same as if you were someone following the titles since at least MGS3.
