Firefly causes crashes in a variety of situations. It's pretty well document, but they've never addressed it.

The only time I've really encountered any sort of issue w firefly crashing the game is with the vendor FWC hand cannon. I have a feeling it's mainly due to LiC procing with the firefly explosion damaging multiple people, but I haven't really been able to pinpoint exactly what caused it, as I've never had any firefly issues w fatebringer/hung jury in all my time playing.

It is the luck in the chamber thing mostly. I know multiple people that have retorted that issue. I also have a buddy that had it happen a couple times with his hung jury when rapidly head shotting thralls.
