On my first character I went to go kill the railroad but couldn't get in. That happened about 2 weeks after release. I just went back last week and reloaded a save over 10 hours earlier to finish the game.

That happened to me as well, it turned out that I had a certain railroad quest activated that wouldn't let the dr. spawn in the location.

I couldn't get into the basement. I was beyond pissed when it happened.

I couldn't either, because you have to kill the doctor to get the key and he wasn't there, so I had to go back like 2 hours and finish all my active railroad quests to fix it and then he was there and I could get in when I tried again.
