Companions never keep up or get stuck. Also, they use explosives at horrible times. The terrible companion AI is why I go alone.

I always sweep Danse's inventory, especially if I hear him say "what's in here?" That part about Valentine using a nuke grenade really happened, lost some serious progress lmao.
