I've had the same problem. Got Destiny with Season Pass back during release, downloaded every expansion as it came out, and got TTK a few weeks ago when some friends wanted me back. I think all you can do is submit that missing VIP rewards form and hope for the best. I haven't heard anything back, so my guess is they don't care. :/
