I have the same issue on my second character that I used the thing to instantly level to 25 and start the taken king quest. I saw another post while looking for answers on this and his character was the one he used the instant level item on as well. I was able to get the sword on a different character though leading me to believe it is due to the item being used

Yes for me and someone else having this issue we had to go to the abandoned quest kiosk and one of the quests there I couldn't tell you which one unlocked it for me
