Does he only spawn mines once during the fight?
If a warlock dies with his poison thing and self revives does that come back as well?

Skolas spawns mines at 50% health and 20% health. If someone dies with the poison there is a little time before he re poisons you. A sunsinger coming back should not be poisoned if he was when he went down.

he spawns the mines twice, once at 50% health and once at 25% health. yes if a warlock dies from the poison they can self res. the thing you really have to bear in mind is just to kill skolas fast, once he is dead all the ads despawn and the poison goes then you are free to disarm the mines.

Cool, thanks. Does the poison come back after a self revive? Does it reappear on the same player?

no it will go to another player but when you self res you will be immune to taking it so the tird player must take it before the other one dies
