I've noticed my titan magnet grenades do the same thing on occasion. Its got something to do with latency and lag on xbox live for me, because the grenades not sticking is usually a precursor to an ark of error codes or just everyone in the lobby red barring.

I look for other symptons though. Gunshot registry prior to and after, character movement, and so on; the issue happens quite often withojt any other connectivity symptoms.
