Problem is; you can't kill other players outside of pvp. So, you're second case never happened.

It happens when you cross, what I assume, are server 'lines' or borders. Like when you follow a random to a different area and they disappear. We were heading to a darkness zone and I stopped to point out a ghost after they crossed into the zone. They came back to where I was and we were invisible to each other until we went further into the mission. We couldn't do any damage to each other though.
