Got mine purely random from one of the sword chaps on the cosmodrome... The knight where the witch is.... (Can't remember her name)
Anyway, redid the level twice straight after and got it again and so did my nephew who came with me from the same knight

Got my first from that same knight. Got my second from a blade of Crota. Dismantled the second though.
