4th void fragment still locked
Completed the raid and went to Ikora, but my 4th fragment is still locked. What am I missing?

For you to run 4 fragments on the Warlock you need to use Child of The Old Gods and Feed the Void. If you’re talking about not having the fragments you need to get them from Ikora.
