just did this with beyond light focusing. Tried for a mask of bakris roll and it gave me a gryfalcons hauberk and a shards of galanor.

I have also had this issue. I don't have any screenshots but I used lightfall armor decryption twice on my warlock and I got one Ophidian Aspect and one Stormdancer's brace, neither of which are from lightfall. I won't focus any more exotics until this has been fixed.
