The reason is that guides often label the pickups in a different order to how they are listed on the triumph. 8 and 9 are the usual suspects because 9 is closer to the entrance to the Gorgon Maze, and 8 is the one near the exit. So people find the last one, then on another run they look up a guide for the location of #9, but it actually shows them #8, the one they already have. So then they think it is bugged. 10, 11 and 12 are similarly affected. The one a guide says is 11, may not be 11 on the triumph. As for asking for screenshots, that'd be really helpful if this needs to be escalated to player support, to see that spots 10, 11 and 12 from a single guide, are all empty. The above advice has been the answer to everyone I have seen with this issue. It has never been a case of an actual missing pickup.

Every guide I've seen says #11 is in the cave on the wall on the way to Atheon after the jumping puzzle. There's no way I'm missing this or mixing up locations. It just will not spawn. Also, I'll load into a fresh VOG just to see if #1 and #2 spawn outside the vault, and those don't spawn either! This bug is really getting on my nerves

Yes, because these guides show the order in which you can get them, many youtubers are just too lazy to show them in order of triumph #11 is when you get to the jump phase after the gorgons and then you go left into the tunnel and jump down The link describes it as #10, which he also says in the comments that this is #11 and not #10
