Can confirm that this is happening to me too, like many others. Can't progress to focus weapons or get catalyst. Have done so many runs with different people thinking it's a quest progress thing, but it's not. Sometimes the panels don't even show up and when they do, it bugges out at the third puzzle, which is nealry after 80% of the dungeon completion. This is absolutely frustrating and demotivating. We need some compensation for all the hours we have spent trying to grind this hoping it will work.

What is the fix? I am currently also having the same issue, I completed the Rogue Network quest from spider, ive done the first and second puzzle, but each attempt (4th attempt so far) ive tried the 3rd puzzle. Each time the same issue persists, exactly as OP describes, shooting the first 4 digits is fine, but after that the last 4 digits are immune to shoot at. Nothing I have tried fixes the issue.

I’m also having this issue but possibly different? I accidentally interacted with the second set of the three puzzles (before the second encounter) before completely the first set in the heart room after the first encounter. On a successive run, I was able to do the first set of puzzles but not the second as the panels had not spawned in. I tried to reload it fresh and the first set of panels did not spawn in either.
