Crow killed her, either on accident... trying to kill Mithrax, or on purpose because she rejected him.
Or Mara did it, for the previous 2 reasons.
Amanda never trusted the light (this is my theory).

I think the Witness might revive her. Remember, what she said... she wasn't scared when she was captured, it was strangely peaceful... like home, like she belonged there (I'm adding on a little bit). Here's why? It's what she told Mithrax, she never really felt at home with the Vanguard, like an outcast... until she met Cayde. It's what she said in Vanilla D2, "... the black hats and the white hats...". She always saw the moral relativity of the dark vs. the light; and saw it as a sham. She was right, btw. Amanda will turn. Who else? If I were the Witness, I want to destabilize the group (Vanguard) that has seemingly murdered all the gods I created. Crow/Mara messed up... the Witness will take Amanda... that in turn will destroy Zavala. These are the cracks that will cause the Vanguard to shatter like glass when Savathun attacks the Witness during the final conflict.

I've played with that theory of The Witness bringing her back with memories intact too just to mess with us. Eris did say during the witch queen campaign the light makes us forget and darkness makes us remember. Now they're chillin' in the Traveler or whatever doing who knows what.
