I am assuming you are using a handcannon. If so, you need to let the reticle reset back to the default state (i.e. where you originally aimed) or else it will appear to fire "ghost bullets", but in reality you are just missing.
Do yourself a favor, go into a private match and line up in front of a wall with your gun. Pull the trigger as though you were in a firefight: if you notice every shot above the one before it, then guess what? You are firing too fast and are not giving the gun time between shots to realign. It is not ghost bullets.

Therefore if u fire a shot it's RNG as to where it goes in a bloom circle around the reticle. Therefore the closer u are the higher the chances of u hitting where at point blank or at optimal range the chances are virtually 100%. However if u are at range your chances are reduced. Imagine it like fallout withVATS as in the further out the less chance of hitting the enemy u have. Therefore when recoil also comes into play the bloom circle shifts upward so less of the enemies hitbox is in the radius that's why bungie have said that the more u let the reticle steady the higher your CHANCES are. Hope I helped it's not ghost bullets it's just u missing but not your fault probably
